Please complete this form if you are an NCIA Board-approved Assisted Living Facility Caregiver Program Coordinator for an Arizona Assisted Living Facility Caregiver Training Program and would like to access TMU©.

Once your application is approved, you can enter new students' training records into the TMU© software and complete their training when they have successfully completed your program. Once a candidate's training record is completed, you will be able to register them for their assisted living facility caregiver medication management exam.
NOTE: If you are a Program Coordinator for multiple training programs, please fill out a separate application for each program.
