No Social Security Number Affidavit Application

If you do not have an assigned Social Security number, please upload your completed No Social Security Affidavit and a form of US government-issued identification.
  •  The US Government-issued ID must be photo-bearing and signed.
  • The FIRST and LAST names on the ID must exactly match the name on the No Social Security Affidavit.
**These must be submitted 72 hours prior to testing date to be processed**
Once you have uploaded the required documentation and submitted this application, D&SDT-HEADMASTER will review your documentation.  If your documentation is the required No Social Security Affidavit and your US Government-issued ID that is photo bearing and signed, we can assign you a unique identification number for testing purposes.
